Home Based Service

VN Home Health Care is a Medicare and Medi-Cal provider of home based services. Our team members include: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work and Home Health Aides. Our staff is caring, knowledgeable and accessible to meet the variety of needs of our patients and their family members. Our qualified staff will coordinate with your doctor to provide care designed to meet your individual needs.

Services are provided with the support of responsive back office staff. Every home care visit provides extensive medical resources to empower patients to live optimally in their preferred home environment.
